It was originally started in 2004 by members of the Brazos Valley Coalition for Life in Texas. The name refers to a repeated pattern of events lasting for 40 days in the Bible, such as Noah's Ark, Moses's 40 days on Mount Sinai, and Jesus's 40 days in the desert.

40 Days for Life is an internationally coordinated effort in over 1,700 cities in 71 countries to pray, fast, and hold peaceful vigils outside of abortion facilities. This united effort has helped save over 25,694 babies from abortion, assist 274 abortion workers who had a change of heart and left their job, and close 172 abortion facilities. The headquarters of 40 Days for Life is the now closed Planned Parenthood where the campaign started in Texas.
